Hoarseness is one of the most common symptoms there is and one of the least often reported, because it nearly always follows something obvious and nearly always settles. The useful discipline is not suspicion — it is a clock. Three weeks is the number clinicians use, and it is worth knowing precisely because the symptom is so easy to keep tolerating.
What makes a voice hoarse without anything being wrong
Viral infections, shouting, long meetings, dry air, smoking, dehydration, and reflux that arrives at night without heartburn — that last one surprises people, because the throat symptoms can appear with no stomach symptoms at all. Some inhalers do it too, and rinsing the mouth after using them fixes a proportion of cases outright.
Swallowing is the more serious neighbour
A sense of food catching, needing to swallow twice, or pain on swallowing is a different symptom from a rough voice, and it is treated with more urgency — particularly when it has been getting steadily worse, when it started with solids, or when weight has come off alongside it.
The lump that is not a lump
A persistent feeling of something in the throat, with normal swallowing and no weight change, is common and usually not a mass at all. It is still worth describing, because the reassurance is worth having and the description is what separates it from the version that needs looking at.
How to use the three weeks well
Rest the voice rather than whispering — whispering strains it more than speaking softly. Drink more water, treat the reflux if it is there, and stop smoking if that is on the table. If it has not settled by three weeks, the appointment is short and mostly consists of somebody looking.
What to raise, and what not to wait on
Worth mentioning at a routine appointment: hoarseness past three weeks, a throat sensation that has lasted longer than that, or a voice that tires by the afternoon. Worth raising promptly: difficulty swallowing that is getting worse, pain on swallowing, coughing blood, a lump in the neck, or hoarseness with unexplained weight loss.
